Handling of Appeals

  1. Appeals will be considered for AHPTS decisions, including the evaluation of participant proficiency testing performance.
  1. A participant shall only be entitled to submit an appeal with respect to a PT program within 20 calendar days of receiving a final or revised PT report.
  1. A participant shall only be entitled to submit an appeal of any other decision within 30 calendar days of receiving notice of the decision.
  1. The appeal must be delivered in writing (by e-mail) to the PT Program Coordinator.
  1. The PT Program Coordinator shall immediately forward the e-mail containing the appeal request to the General Manager.
  1. General Manager in consultation with Quality Manager, Technical Manager and PT Co-Ordinator will study the contents of the appeal carefully to determine whether they are valid or not.
  1. AHPTS ensures that decisions on appeals are made by, or reviewed and approved by, individuals who were not involved in the original decision that is the subject of the appeal
  1. If the appeal is found to be invalid, the appellant shall be advised of this and the reason for the rejection and the appeal shall be deemed to be closed with no further action being required.
  1. If the appeal is found to be true, appropriate actions will be taken in accordance with the Control of Non-conforming work (QP/15).
  1. If the appeal is related to the contents of PT report and found to be valid, the appellant and all other participating Test Laboratories and all interested third-parties shall be notified within 48 hours by e-mail that the PT Report has been withdrawn pending correction or amendment (as applicable).
  1. After correction and or corrective action has been implemented, the effectiveness of the actions taken will be evaluated by the Quality Manager/Technical Manager.
  1. Upon the determination that the actions are satisfactory, PT Co-Ordinator will notify appellant of the action taken and close the appeal.
